Breast augmentation is a popular procedure which may help patients improve their self-confidence and feel more comfortable in their bodies. However, with breast augmentation may eventually come the need for breast implant revision. While there is no set timeline of when a patient may require a revision, there are some signs that it may be time for one.

What is a Breast Implant Revision?

Breast implant revision is a procedure in which a surgeon replaces existing implants with new ones. Other goals of this procedure may include correcting capsular contracture, addressing rupture, changing implant size, or changing the type or texture of the implant.

This procedure may involve releasing or removing scar tissue around the implants, repositioning, or adjusting the implant pockets. The procedure is similar to breast augmentation surgery, although as with any revision surgery comes added complexity.  It is important to determine whether the same or different incision will be performed and whether the implants will replaced in the same plane (above or below muscle), or if a change of plane may be recommended.

Causes for Breast Implant Revision

There are several issues with may lead to breast implant revision surgery, including:

  • Capsular contracture: This is an immune system response to foreign material (breast implants) in the human body. Fibrous scar tissue will form around the implant, creating a tissue capsule. This capsule is usually soft and helps to keep the implant in place, but for some women, the capsule may become hard and dense.  This may change the appearance of the breast and potentially lead to discomfort in some patients.
  • Wrinkling/rippling: Implant wrinkling or rippling occurs when the edges or fold of the breast implants can be seen through the skin.  It is more common in relation to saline implants, particularly in thin patients.
  • Rupture or leakage: Implant leakage or rupture may be caused by abrupt physical pressure or due to the weakening of the implant shell over time.  Rupture of a saline implant will deflate the implant resulting in a visible difference in size.  Silicone implant rupture is often more subtle and may only be apparent on breast imaging.
  • Shifting of the implant: Breast implants may slightly shift position over time.  This may be related to the long-term effects of gravity and implant weight.  Shifting may also be appreciated following pregnancies and breastfeeding.

The most common reason women choose breast implant revision is that they are unhappy with their current implant size or shape.

Many women who chose breast implants before having children may have experienced significant changes in their breasts during pregnancy. Because of this, they may wish to have their breasts restored to a more youthful shape through breast implant revision.
