Power Plastic Surgery ("we", "us", "our") is the cosmetic plastic surgery practice of Dr. Stephanie Power, MD, MSc, FRCSC, located at 7 St. Thomas Street, Suite 602, Toronto, Ontario M5S 2B7. Protecting the privacy of our patients and website visitors is part of the care we provide.
This policy explains what personal information we collect, how we use and share it, how we protect it, and the choices and rights you have. It applies to information we collect through www.powerplasticsurgery.com, by phone and email, through our online booking service, and in the course of your care at our practice.
Personal health information is governed by Ontario's Personal Health Information Protection Act, 2004 (PHIPA). Other personal information, such as information collected through our website before you become a patient, is handled in keeping with the federal Personal Information Protection and Electronic Documents Act (PIPEDA).
Your Personal Health Information
Dr. Stephanie Power is the health information custodian for patient records at Power Plastic Surgery. Our staff and service providers act as her agents and are bound by confidentiality obligations.
When you become a patient, we collect the information we need to assess you and provide safe care. This can include your contact details, date of birth, health card number, medical and surgical history, medications, allergies, consultation notes, clinical photographs, and information from other health care providers involved in your care.
We use and disclose this information to:
- assess you, plan and provide your treatment, and follow up after surgery;
- communicate with other health care providers and facilities involved in your care, such as your family doctor, an anesthesiologist, a laboratory, or the surgical facility;
- schedule appointments and send reminders;
- process payments, deposits and financing you arrange;
- meet our legal and professional obligations, including those of the College of Physicians and Surgeons of Ontario; and
- maintain and improve the quality and safety of our care.
We rely on your implied consent to share information with other providers in your circle of care unless you tell us not to. We ask for your express consent before using or disclosing your health information for any other purpose, except where the law permits or requires it.
Clinical Photographs
Before and after photographs are part of your medical record. We will never use your photographs for education, our gallery, our website or social media without your separate written consent. You may decline without any effect on your care, and you may withdraw your consent for future use at any time.
Information You Give Us Online
When you request a consultation or contact us through our website, we collect your name, phone number, email address, the procedure you are interested in, and any message you choose to include. When you sign up for our newsletter, we collect your email address. When you book online, the booking service collects the details needed to schedule your appointment and any deposit.
We use this information to respond to you, answer your questions, schedule appointments, and, if you have agreed, send you news and offers. Please do not include detailed medical information or photographs in a website form; our team will collect what is needed securely once we are in touch.
We do not sell, rent or trade your personal information.
Service Providers and Where Information Is Stored
We use trusted service providers to operate our practice and website. They may only use your information to provide services to us and must protect it. These include:
- HubSpot, our customer relationship management system, which receives website enquiries and manages our email communications;
- Vercel, which hosts our website, and Microsoft Azure, which stores our website's images;
- Google, which provides website analytics and advertising measurement; and
- our email, online booking, practice management and payment providers.
Some of these providers store or process information outside Canada, including in the United States, where it may be accessible to authorities under the laws of that country.
Cookie Policy
Cookies are small files that a website stores on your device. Our website and the tools it uses set cookies and similar technologies for the following purposes:
- Analytics: Google Analytics helps us understand how visitors find and use our site, such as which pages are viewed. We do not use it to identify you.
- Advertising: Google Ads tags measure whether our ads lead to calls or consultation requests, and may be used to show our ads to people who have visited our site.
- Marketing and enquiries: HubSpot tracking helps us connect a consultation request with the pages that led to it, so we can respond helpfully and improve our website.
- Site features: our accessibility menu (UserWay), web fonts (Adobe Fonts and Font Awesome) and sharing buttons (AddToAny) load from their providers, who may receive standard technical information such as your IP address and browser type.
You can block or delete cookies in your browser settings, and the site will still work. You can also manage how Google uses information for ads at adssettings.google.com and install the Google Analytics opt-out add-on.
Email and Marketing Communications
We send newsletters and promotional messages only with your consent, as required by Canada's Anti-Spam Legislation. Every marketing email includes an unsubscribe link, and you can also ask us to stop at any time. Appointment, care and billing messages are not marketing and will continue while you are a patient.
How We Protect Your Information
We use administrative, technical and physical safeguards appropriate to the sensitivity of the information, including restricted access, staff confidentiality agreements, password-protected systems, and encrypted connections on our website. No method of transmission over the internet or by email is completely secure, so we ask that you not send sensitive health details through website forms or regular email.
If your personal health information is stolen, lost, or used or disclosed without authority, we will notify you as required by PHIPA.
How Long We Keep Information
We keep medical records for the periods required by law and by the College of Physicians and Surgeons of Ontario, which for adult patients is at least 10 years from the last entry in the record. Website enquiries from people who do not become patients are kept only as long as reasonably needed to respond and follow up, and are then deleted or de-identified. Records are destroyed securely.
Your Rights and Choices
You may:
- request access to the personal health information in your record, and ask us to correct information you believe is inaccurate or incomplete;
- withdraw your consent to certain uses or disclosures of your information, subject to legal limits;
- ask that we not share information with a particular provider in your circle of care; and
- unsubscribe from marketing messages at any time.
We will respond to access requests within the time required by law. A reasonable fee may apply for copies of records, as permitted by PHIPA. To make a request, contact us using the details below.
Social Media and Other Websites
Our website links to our social media pages and to other websites, which have their own privacy practices. Comments and messages on public social media are visible to others, so please do not share health information there. We will never discuss your care publicly, even in reply to a review.
Changes to This Policy
We may update this policy from time to time. The date at the top shows when it was last revised.
